Travel Vaccinations
Please note that not all vaccinations are available free of charge through the NHS - ask the receptionist to confirm the charges for the vaccination you require, if any.
Please complete the below form, print it off and bring it in or post to the surgery. The Practice Nurse will then give her recommendation as to which, if any, vaccinations you may need. You will need to call the surgery to check when this has been completed.

You can also request the above form at Reception. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make the initial appointment as early as possible - at least 8 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
IF YOU ARE PLANNING TO TRAVEL ABROAD, PLEASE CONTACT THE SURGERY AT LEAST 8 WEEKS IN ADVANCE TO FIND OUT WHAT HOLIDAY VACCINATIONS YOU REQUIRE AND TO SEE IF WE HAVE THE VACCINES AVAILABLE TO YOU.
IF YOU ARE TRAVELLING IN LESS THAN 8 WEEKS, YOU WILL NEED TO CONTACT A PRIVATE CLINIC FOR YOUR VACCINES.

The nurse will review the information you have provided and make the appropriate recommendations for travel vaccinations and/or medications.
