Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.
You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.
Your choice will only apply to the health and care system in England. This does not apply to health or care services accessed in Scotland, Wales or Northern Ireland.

Access to your medical record (the Right of Access)
You have the right to get a copy of the information that is held about you.
This is known as a subject access request.
Please note:
- You can sign up for secure online access to your full GP electronic record.
- You can then look at your medical record whenever you want.
If you do wish to make a subject access request then:
- You can do this in writing (letter, email, fax)
- You can make such a request verbally to a member of staff or a doctor or nurse that you are consulting with at the surgery
Please let us know exactly what information you would like and we will provide you with the information within 28 days. Proof of ID will be necessary before releasint any medical records.
