Community Gateway
Community Gateway - Cornwall Link (cornwall-link.co.uk)
The Community Gateway is for everyone and provides a single point of access to a wide range of voluntary sector services, activities and support. It will provide a ‘gateway to independence’, working with people to identify what matters to them - and offer appropriate packages of support.
The Gateway aims to create personalised plans that mitigate admissions, reduce social isolation, and improve wellbeing. It will also connect people to mental health support, community activities, the community hub network and wider winter support including hot food and warm spaces.
The Gateway can be accessed by anyone via a dedicated telephone line (01872 266383) and is open 8 am to 8 pm, seven days a week. In addition, staff members in the community will be available to offer one-to-one and group support. Alternatively, you can email: gateway@ageukcornwall.org.uk

Some minor injuries can be dealt with here at the surgery, however if you have an injury that requires an x-ray it is best to go straight to a Minor Injuries Dept or Casualty at Royal Cornwall Hospital Treliske.

British Red Cross - Wheelchair Hire/ Medical Aids
The Mobility Aids centre at Tolvaddon is now back open (March 2022). Opening hours are 09.00 am to 1.00 pm Monday to Friday.
They hire out wheelchairs and toileting aids to anyone that needs them and they also have assisted daily living products for sale. For hire charges, or any other questions, please give them a call on the number below or have a look on their website www.redcross.org.uk for further information.

Carnon Downs Village Hall
Carnon Downs Village Hall has had extensive rebuilding and refurbishment work carried out this year thanks to a substantial grant from the Big Lottery.
There are two brand new halls and a meeting room, providing space for a wide variety of events.
CDVH is managed by a Volunteer Committee of Trustees made up of Local Residents and Representatives of Groups that regularly use the hall and it is a community organisation run for the benefit of local residents.

About the Role
Alzheimer’s Society Cornwall Dementia Support Service involves dedicated members of staff working with people with dementia and/or their carers. We:
- Support people to maintain their independence, improving their sense of well-being, and help them feel in more control of their lives.
- Enable people to access other provision by providing supported referrals into relevant services
- Build relationships with a range of local contacts and network with health and social care professionals
- Help to organise and facilitate peers support groups that inform, educate and support those affected by dementia e.g., Singing for the brain (Truro)
- Provide support face-to-face, on the telephone and online.
- We are community based, working from home, out in the community and meet people in the place they choose, at home or in their community.
Our people are passionate about putting people affected by dementia at the heart of everything we do, to make our service the best it can be.
Our online support includes Talking Point, our online community where you can connect with others in a similar situation, Dementia Talking Point is a helpful online community where anyone who is affected by dementia can receive valuable support. It's free, open day or night, and can be accessed online and our dementia directory where you can search for local services.
Dementia UK - Admiral Nurses - How can they help?
What is an Admiral Nurse?
Admiral Nurses are specialist dementia nurses who are supported and developed by Dementia UK. They provide lifechanging support for families affected by all kinds of dementia, including Alzheimer’s disease.
Admiral Nurses provide families with tailored clinical advice and psychological support. They have the time to listen and the knowledge to solve problems. They are a lifeline for people with dementia, their families and friends.

Home Treatment Team
The team work 24 hours a day, 7 days a week to support people in crisis. When you call the service you will be asked for your name, address, date of birth and the reason for your call. If you call at between 8.00pm and 8.45am there may be a delay in answering some calls, but they will repsond as soon as they can.
When calling out of hours they will access your electronic care plan if it is available and your care co-ordinator will be informed of your call the next working day.
If the line is engaged you may be asked to leave your details with the Bodmin Hospital switchboard.

Message in a Bottle
Message in a Bottle is a national emergency information scheme distributed by Lions Clubs in the UK, it provides the Emergency Services with vital life saving information.
They are available FREE OF CHARGE giving peace of mind to both the recipient and their family.
A readily available container known to the emergency services contains vital information about the patient and is placed in the fridge. Labels are placed on the fridge door and on the inside of the front door which notify the emergency services that the patient is part of the scheme.
Information includes:
Personal Details, Medical Condititions, Doctors Details, Location of Medicines, Pets, Carers/Health Visitor Details, Dependants and Family Contact Details.

MSI Reproductive Choices is a well-established sexual health charity. They are proud to be providing services to communities in Cornwall and across the South West region to offer patients choice. Their aim is to help make it straightforward for women to find the information they need about abortion care and to be able to self-refer for a consultation as easily and conveniently as possible. Supportmatch Homeshare Pilot Scheme
Supportmatch Homeshare is a not-for-profit organisation, they have partnered with Cornwall Council and the Cornwall Voluntary Sector Forum (VSF) to deliver a Homeshare Scheme in Cornwall. Supported by Kate Shields, CEO Integrated Care System and Rachel Wigglesworth, Director of Public Health.
The Scheme provides live-in support to adults by matching people who need live-in support (the ‘Householder’) with those that can give it (the ‘Homesharer’). More specifically, the Scheme involves matching someone with a spare room that would like practical support around the home and companionship with someone that needs affordable accommodation and can provide practical support and companionship for an agreed number of hours per week.
The benefits are significant and can improve quality of life, for both the Householder and the Homesharer, through reducing loneliness and provide practical support such as cooking and helping prepare meals, basic meals, general housekeeping and provide an overnight presence for both parties (note: this arrangement does not include personal care).
Typically support and companionship is between 10-15 hours per week and the accommodation provided is very affordable, safe and secure. Both parties are vetted and safeguarding is a crucial element of this Scheme. If you, your colleagues, family/friends, as a Householder, has a spare room and want to learn more about Homeshare and the many benefits of having a Homesharer provide you with live-in companionship and support, we can help!
