Social Prescribing aims to connect people to activities, information, and resources. This is a chance for you to speak to someone in a confidential setting about your life and the things you might be struggling with. An appointment with a social prescriber can provide practical and supportive guidance to figure out what is important to you and find who can help.
What can I help with?
- Getting involved in local groups, social activities and support groups
- Accessing befriending services
- Signposting you to support around housing, benefits, and financial matters
- Helping with accessing transport options
- Finding out about volunteering opportunities
Social Prescribing is available through the Canford Heath GP surgery to anyone over the age of 16.
Who can I support?
- Low in confidence or self-esteem
- Long-term conditions
- Socially isolated - having little or no support
- Unemployed - out of work or unable to work owing to sickness
- Those experiencing bereavement or loss
- Carers or ex-carers
- People interested in finding new hobbies or interests
- People struggling with low mood or anxiety
