Surgery Closed - Thursday 5th December 2024 at 1.00pm

The surgery will be closed on Thursday 5th December 2024 at 1.00pm and will re-open on Friday 6th December at 8.00am. Please attend the surgery as normal if you have a pre-booked appointment for your Flu and Covid Vaccine.

If you have a medical emergency, such as chest pain or difficulties breathing, please hang up now and call 999.  

If you require medical assistance before 6.30pm this evening, and this cannot wait until the practice reopens tomorrow morning, please call 0300 123 5491. If you need medical assistance after 6.30pm this evening, please log onto NHS 111 online or call 111.

Statement of Purpose

Dr Jane Greig is the appointed Registered Manager at Moorland Medical Centre and is the point-of-contact for registration of the service with CQC and all further CQC-related matters.

A statement of purpose is a document which includes a standard required set of information about a service. Statements describe:

  • The provider’s aims and objectives in providing the service.
  • The kinds of services provided.
  • The health or care needs the service sets out to meet.
  • The locations where the services are actually provided or provided from.
  • Details about the provider, including their legal status, and any manager(s), including the ‘address for service’ for all registered persons
