New Patient Registration

We are amending the patient registration process to reduce the spread of Coronavirus by limiting the volume of traffic to the Practice and reducing the amount of paper handled.  Please complete an online registration form found on the next tab this can be sent directly to the Practice by using the send button.

Currently we offer a combination of Face to Face and Telephone Consultation appointments with a GP

If you prefer a particular type of doctor e.g. female for a female patient and vice versa we will note this and do our best to respect your choice. However not all doctors in the practice provide all service and specific doctors may not be immediately available.

Practice Area

Our area comprises the postal districts of:G21, G31.1, G31.2, G31.3 and G33 with the exception of G33.6.

Online 'Pre-Registration Form'

For your convenience you can complete a pre registration form online.  We will hold these details on file. When you visit the surgery to finalise registration you will be asked to sign the form to confirm that the details are correct and show proof of residency. You will be offered a New Patient Medical appointment with our Health Care Assistant.

If you wish to complete the pre-registration form click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
