Patient Registration

Members of the public who reside within the practice area * may apply for inclusion on a practice list.  

Please note each practice at the Surgery provides General Medical services for a geographical area extending from the Surgery address.

* Practice area refers to:

SCRABOVIEW PRACTICE Dr  Youngs, Kirk & McIlroy- 5.5 mile radius with a boundary map agreed by SPPG

CHURCH SIDE PRACTICE Dr Hyland, Rankin Logan & Armstrong - 5 mile radius

Please contact Reception if you wish to register at the Surgery and live within the practice area. You may collect a Registration application pack at Reception.

All relevant forms and required registration documents must be returned to the Surgery to ensure your registration will be completed. 

PLEASE NOTE HSC HAVE PUT A TEMPORARY FREEZE IN PATIENTS MOVING WITHIN THE AREA UNTIL MARCH 25.
