PATIENT REGISTRATION
Before a patient can register at the practice the patient must provide identification. Confirmation of this identification is required by the Health Board. A registration form is completed to allow the practice to request medical records from the patient’s previous surgery. A medical questionnaire also needs to be completed as it can be a few weeks before we receive your medical records.
We will ask you to sign a mandate which will allow us to contact your previous GP surgery for basic health information.
Patients on medication should ensure they have an adequate supply from their previous surgery as we may prefer not to prescribe until after we have received your medical records.
All New Patients aged 15 years and over are given an appointment with the Healthcare Support Worker within the first month for a new patient medical, which includes a blood pressure check. Patients aged 5 – 15 years will only be given a New Patient medical if requested by the patient or the patient’s parent or guardian, if the request is made within the first 6 months of the patient being registered. Children under 5 years of age do not need a medical. At this appointment measurements of height, weight , blood pressure and urine tests are taken.
We are a busy medical practice with over 11,000 registered patients. If you cannot keep your appointment please cancel to allow someone else to use this appointment.
IMPORTANT INFORMATION FOR NEW PATIENTS AND TEMPORARY RESIDENTS
The Practice policy in relation to drugs which can be abused or related to addiction problems is detailed below.
The Practice will not issue the following drugs to new patients or temporary patients:-
- Methadone
- Dihydrocodeine
- Diazepam
- Tramadol
- Solpadol
- Hypnotics i.e. sleeping tablets and other potentially addictive medications will be reviewed.
We believe that such medications should only be supplied by the specialist addictions services or your registered GP.
Registration of Children Under 6 Years
It is essential that the practice receives accurate information regarding childhood immunisations. A recall system is in operation to remind parents of when immunisations are due.
Change of Address or Telephone Number
Please let us know as soon as possible if you change your address or telephone number. There is an online form which you can access to keep us informed of any changes.
