Our team consists of two experienced GP partners—one male and one female doctors—who hold surgeries throughout the day, every weekday. Together, we offer a full range of medical services tailored to the needs of our patients.

How Does PATCHS Work?

You can access GP services online with PATCHS in just four simple steps:

You can use PATCHS by:

• Clicking the PATCHS banner above

• Using the NHS App 

✅ 2. Register for PATCHS

There are a few ways to register:

• Use your NHS App or NHS Login

• Create a PATCHS account directly

• Use a Guest Account (no registration needed)

Note: Guest users must re-enter personal details each time and cannot view previous requests.

✅ 3. Log In to PATCHS

Once registered, log in using your email address and password.

✅ 4. Submit Your Request

Choose the type of request and answer a few quick questions. Your answers go directly to the GP practice.

• A clinician will review your request during opening hours

• You may receive a response by online message or telephone

• Face-to-face or video consultations will be arranged if needed

Why does the receptionist need to ask what’s wrong?

Our reception team are an important part of our practice. They are trained to ask a few questions to help make sure you get:

• The most appropriate medical care

• From the right health professional

• At the right time

Please be assured that reception staff are bound by strict patient confidentiality and all information you share is handled sensitively.

Test Results

Please call after 1pm to get your test results or to check if any items are ready for collection.

To protect your confidentiality, test results will only be given to the patient concerned unless prior permission has been granted or the patient is unable to understand the results.

When you have a test, you will be informed how long it will take for the results to return to the practice.

It is your responsibility to check your results and make an appointment with your doctor if advised.

New Service – NHS Pharmacy First

Starting 31st January 2024, your local pharmacy will offer a new NHS service to provide advice and treatment for common health conditions.

Pharmacists can assess your symptoms and supply NHS medicines, including some prescription-only medicines, without needing to wait for a GP appointment.

This service covers the following conditions:

• Sinusitis

• Sore throat

• Earache

• Infected insect bites

• Impetigo

• Shingles

• Uncomplicated urinary tract infections (in women aged 16–64)

If you have any of these symptoms, simply visit your local pharmacy for an assessment and, if appropriate, treatment.

🤰 Maternity Circles – A Safe Space for New & Expecting Parents

We’re excited to introduce Maternity Circles – welcoming, drop-in sessions designed for pregnant women and parents with children up to 2 years old.

These sessions offer a safe, warm, and friendly space where you can:

• Enjoy free refreshments

• Access support from professional services

• Get useful information about pregnancy and early parenthood

• Build connections with other parents

• Receive support for dads, carers, and co-parents

Skills House – Support to Get Into and Stay in Work

Skills House offers tailored support to help people move into employment and stay there.

The service is available to individuals who are currently out of work, including those with disabilities, health conditions, or other non-health-related barriers that may be affecting their ability to find employment.

• Out of work? You can receive up to 12 months of support to help you secure a job.

• Already in work but struggling? You can access up to 4 months of support to help you manage and sustain your employment.

Support is personalised to your individual needs. An Employment Specialist Adviser will provide dedicated one-to-one support, helping you to set clear goals and work towards achieving your long-term aspirations.

Whether you need help building confidence, overcoming barriers, or maintaining your role, Skills House is here to support you every step of the way.

Practice Nurses

Our Practice Nurses runs our health promotion clinics including Asthma, Diabetes, Coronary Heart Disease and Smoking Cessation.

Our Practice Nurse also undertakes cervical screening, contraception and immunisations.

Maternity Services

All doctors provide maternity services. Antenatal appointments are arranged at times to suit the patients and doctors.

Contraception

The practice offers a comprehensive service within normal surgery hours.

Child Health & Immunisation

The health visitors work in the practice child health clinics and in the community assessing the health needs of the young, elderly and disabled. They are generally available from 9.00am to 5.00pm but messages can always be left for them by telephoning Bradford 322012 for patients in the Wibsey area or Bradford 322027 for patients in the Buttershaw, Westwood Park area.

Domestic Abuse 

If you need support, this might mean you feel unsafe at home or someone you love makes you feel frightened. Please let the receptionist know that is why you are calling the practice if you can. Some services we provide are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

Our Patient services will advise of the fee at the time of your request. (We do accept card payments). 

First Contact Physio (FCP)

If you have muscle or joint problems, back pain, or strains, you can be seen by a First Contact Physiotherapist in one of our extended access clinics.

• The FCP will assess your condition and decide the best treatment pathway.

• They are specialists in musculoskeletal (MSK) conditions.

• You can speak to one of our advisors to arrange an appointment.

• The FCP can manage your condition or refer you for further care if needed.

Social Prescribing

Social prescribing connects you to support for non-medical issues that affect your health and wellbeing. They can help with:

• Housing and benefits

• Employment/unemployment

• Debt management

• Domestic violence and abuse support

• Language support

• Social isolation and befriending

• Community groups

• Bereavement support

Minor Eye Conditions Service (MECS)

MECS offers quick access to help for new and minor eye problems, closer to home.

Inclusion criteria (conditions treated):

• Vision loss (including transient loss)

• Eye pain

• Red eye diagnosis

• Foreign body removal (except contact lens fittings)

• Dry eye, blepharitis, epiphora, trichiasis

• Lumps/bumps near the eye

• Recent onset flashes and floaters

Exclusion criteria (conditions NOT treated):

• Severe eye conditions needing hospital care (e.g., orbital cellulitis)

• Herpes zoster related eye problems

• Adult squints or double vision

• Suture removal

• Symptoms needing a full sight test

• Suspected eye cancers

• Age-related macular degeneration

• Repeat MECS visits for same condition within 3 months

Dentist

Accessing NHS dental care in an emergency or out of hours:

• Dentists offer urgent and routine treatments.

• If registered with a dentist, contact them by phone (you may need to wait if not urgent).

• If you don’t have a dentist and need emergency dental care, call NHS 111 or find an NHS dentist via their service.

Making a Complaint

How to Raise a Concern

Most issues can be resolved quickly by speaking directly to the person involved. If this does not work or you want to make a formal complaint, please do so in writing as soon as possible, ideally within a few days, and no later than 12 months after the incident or from when you became aware of it.

• If you are a registered patient, you can complain about your own care.

• To complain about someone else’s care, you normally need their written permission.

How to Submit a Complaint

You can request a complaints form (including a third-party authority form) at reception or write your complaint yourself, ensuring all necessary details are included.

What Happens Next?

• We aim to acknowledge your complaint within 5 working days.

• We will aim to resolve it as quickly as possible and will inform you if it will take longer.

• You may receive a written response or be invited to a meeting.

• We investigate what happened, why, and seek to learn from it.

• If your complaint involves more than one organisation, we will coordinate responses with your consent.

Complaints on Behalf of Someone Else

• Written consent from the patient is required unless the patient is deceased or unable to consent due to illness.

• Ask at reception for a complaints form with authority or request one be sent to you.

• If the patient is capable of complaining themselves (e.g., a child), they should normally contact us directly.

• We may still need to communicate directly with the patient depending on consent.
