Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment early with the practice nurse to discuss your travel arrangements and bring with you a completed travel assessment form. (Travel Assessment Form 0816)
This initial appointment must be at least 6 weeks before you travel as some destinations require a course of vaccinations to be given.(if it less than 6 weeks before departure you must go to a private travel clinic).
Some travel vaccinations are not provided by the NHS. Colinton Surgery however is willing to provide many of these vaccines but please be aware that there is an additional charge for these to be administered. (see travel assessment form) The cost for the full vaccination course must be paid in full at the time of the first vaccination.
This charge covers the cost of the travel clinic consultation, providing and administering the vaccination and completing any additional paper work that is required (e.g. provision of yellow fever certificate).
Please note: failure to pay for the complete course of vaccination will result in future vaccinations being refused which may result in you not being covered in time for your trip and impact your travel insurance.
