New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.
Acceptance onto our list is at the discretion of the doctor. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Please bring a small sample of urine when you attend.
Medical treatment is available from the date of registration. Please contact reception for further information.
Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

When accepted please make an appointment to see our practice nurse for a New Patient Medical – your registration will not be complete until you have attended this medical.
Please bring a specimen of urine for testing (a bottle will be provided on request) and also any medication you have been taking.
If you would rather register with a paper form then Download, print, complete and deliver to the practice in person between 17:00pm - 18:00pm
Change of Address or Telephone Number - If you change your telephone number or address, please let us know as soon as possible in case we need to contact you in an emergency.
