To complete the registration, please come in to reception with the completed registration form and your baby's Red Book or birth certificate.
We only register children under 16 years of age who do not have a parent registered with us in exceptional circumstances.
If you have trouble downloading or collecting the forms, please ring us to arrange for us to post the forms out to you.
