New Patient Registration

To join the practice we ask that you live within the practice catchment area. Our practice catchment postcode area G64 1 , G64 2 , G64 3, G21 3 and parts of G33 1 (Robroyston). If you have any queries please contact the practice.
We will require you to fill in a GPR1 application form and a patient questionnaire both linked below. Please provide two items of identification:
- Personal identification i.e Drivers license/passport
- Proof of address i.e. Utility bill.

Patient Questionnaire
You will be asked to attend for a new patient medical with the Health Care Assistant at which time you will be asked for details and dates of illnesses, operations, tests, vaccinations, medications etc. It can often take some time to transfer your medical records from your previous GP therefore it is essential to conduct this assessment. If you are on any repeat medication please obtain a copy of your prescription before leaving your old practice, this ensures we can provide you with any medication required.
Change of personal details
If you change any personal details at all (e.g. name, address or telephone number) we would appreciate being notified of these changes as soon as possible so that we can update our records.
