BLOOD PRESSURE MONITORING AT HOME

Sometimes, we may ask you to record your blood pressure at home.   This gives us a series of readings over one week, from which we can calculate an average blood pressure.  This is often a more reliable figure than taking just one reading in the surgery which may be artificially elevated due to 'white coat syndrome' i.e. a degree of stress that comes purely from the fact one is 'at the doctors'.

A) Blood Pressure form for completion at home: 

Home Blood Pressure Form and Instructions  You can either:

     1.  Print off the form, complete it, and then bring it in to the surgery with any loaned equipment 

                                                    OR (if you have your own blood pressure monitoring machine): 

     2.  Download the form and save on your computer, and then email the form back to us at: somicb.westcokersurgery@nhs.net  (you will need to 'enable editing' in order to complete the form electronically).
