Patchs is a easy way to contact your GP practice online.

All you have to do is register once then you can submit a request for yourself or on behalf of someone you care for.

Please note: Patchs is only available during surgery opening hours and will not be covered over the weekend, bank holidays or evenings.  If you require medical attention,

please ring 111 or visit https://111.nhs.uk/   

If you have a life-threatening emergency, please ring 999.

Are you a Military Veteran?

A veteran is someone who has served in the armed forces for at least one day. 

Being flagged as a veteran in your NHS medical records will help to ensure that you are able to access dedicated services for those who have served in the UK Armed Forces.  These include services for Mental Health and Physical Health conditions.

Try the NHS App 

If you're a patient who are aged 13 or over at Fountains Medical Practice, you can now use the NHS App, a simple and secure way to access a range of NHS services on your smartphone or table. 

The NHS App allows you to: 

  • View and share proof of your Covid-19 Status 
  • View your vaccination details and download PDF vaccination certificate 
  • Check your symptoms to get health advice
  • Find out what to do when you need help urgently
  • Request repeat prescriptions
  • View your GP record securely
  • Register to be an organ donor
  • Choose how the NHS uses your data
  • Book & manage appointments at the Surgery

The NHS App is really easy to use and it is an effective alternative to our current Patient Access Online Services system.  It is available free to download for iOS from the App Store & for Android from Google Play. If you are having any problems using the NHS App, please visit the website to contact their team directly https://www.nhs.uk/contact-us/nhs-app-contact-us 

Unlike our current Patient Access system, you can even prove your identity using the App itself without needing to bring any ID to the surgery.  If you are already a user of Patient Access, you will be able to access exactly the same information on the NHS App as you currently do on Patient Access.

Choose if data from your health records is shared for research and planning 

Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.

You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.

Your choice will only apply to the health and care system in England. This does not apply to health or care services accessed in Scotland, Wales or Northern Ireland.
