Fit Notes (Sick lines)
Please be aware that the first 7 days ( including non working days) of your absence from work are covered with a self certificate. Your employer will normally have a self certificate form for you to complete. If not, you can access a generic form here

GP are unable to complete a fitnote before 7 days.
If you are absent for more than 7 days then you can request a Fitnote by completing the form below and emailing it to gram.inschadministrator@nhs.scot

If you require an extension to you initial fit note, then this can be requested in the same way. Please note that Fitnotes cannot be pre-dated and can only be created on or after the start date.
