How to download the NHS App
1. Open the App Store (for iPhone or other IOS devices) or Google Play (for Android).
2. Search for "NHS App"
3. Select "Install"
4. Open the App
To register for the NHS App you must:
- Be at least 13 years old
- Have an email address and phone number
- Be registered with a GP surgery in England
To register
1. Enter your email address and select "Continue"
2. Choose a password
3. Accept the NHS login terms and conditions
4. Enter the security code sent to your email address.
5. Enter your phone number and select "Continue"
6. Enter the security code sent to your phone.
John Hopkins Segmentation PNG's - Patient Need Groups
You may notice a test result that appears in your NHS app referring to John Hopkins Score (Patient Need Groups)
PNG's (Patient Need Groups) are a simple way of categorising patients based on their specific health needs. It helps us as a Practice to understand the individual needs of our patients based on their health and wellbeing needs. This will enable personalised care and ensure patients get the relevant support.
PNG's are formed using a well-tested set of 11 segments/groups based on research by John Hopkins University. Factors used for grouping include the number of conditions a person has and how severe they are. This helps in deciding what level of support a person may need.
The categories breakdown into :
- Low need (PNG 1-4) - people without significant health issues who need less intervention. Preventative care may stop them becoming unwell.
- Moderate need (PNG 5-9) - people who have a condition that is well managed. They may need occasional supprt to remain stable.
- High need (PNG 10-11) - people who have several conditions. They may need regular monitoring or a more coordinated approach.
