New Patient Registration
If you live within our practice area you are welcome to register with us. You will need to complete a GMS1 Family Doctor Services Registration Form, which you can obtain from reception. You will also be asked to complete a health questionnaire form. If you outside the practice area we may register but please come in and ask us first before completing any forms.
Newly registered patients are invited for a health check with the practice nurse. You may express a preference regarding which doctor you would like to see.
Medical treatment is available from the date of registration. Please contact reception for further information.
We are happy to register patients who are asylum seekers or refugees, people from travelling communities or people who are homeless. Please enquire at reception and we will help you and your family to register with us. We also have leaflets explaining the registration for specific groups.
Changes to Personal Details
Please inform us if you move house, change name or telephone number. It is important that we can contact you in connection with test results, referral information or invitations for procedures or immunisations that are due. If you move outside the practice area you will be requested to register with a more local doctor.

Patients with a temporary address
If you are staying in the area on a temporary basis we can still register you for temporary care. You must come into surgery with some ID if possible and we will temporarily register you so you can get the help required.
If you are homeless, sofa-surfing, staying with a friend,or a gypsy/traveller you can still register either temporarily or with a full registration. If your circumstances are difficult or you have no ID please ask for the practice manager and she will be able to support you with registration.
