New Patient Registration

To apply to become registered with our practice, you need to request a new Patient Registration Request Form

In order to register with us, you are now required to provide 3 items of supporting identification with your application.  We have a list of approved supporting documents at reception desk, so please ask.

TEMPORARY RESIDENTS

Anyone staying or working temporarily in the area and in need of medical attention or advice is welcome to make an appointment to see the doctor or nurse.  N.B. It is not essential to have your medical card with you, however you will need to complete a temporary resident form.

 PATIENT'S CHANGING ADDRESS

As of 1st April 2016, patient's who move to a new address outside the practice area of 8 miles, will be asked to move to a GP practice nearer their new address.
