The breast screening unit which was in Poringland in the spring has now left the area.  For patients that missed their appointment or are outside the screening age but can request screening once every three years, should call the screening team on 01603 287777. 

Registration at Heathgate Medical Practice

We are now using a new national online service called 'Register with a GP surgery' which makes registration with us easier.  Capturing the registration details electronically allows us to process your registration quicker and we will send a SMS text when this is complete.  

Click here to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe.  It cuts our administrative workload and makes it easier for you to register.  If you are unable to use the digital link, you can down load a paper version of the form here.  This should be printed, completed and returned to Practice.

In either cases, if you currently take repeat medication and would like us to review the continuation of this, you should provide us with a copy of the repeat medication listing from your former Practice.
