HOW DO I REGISTER WITH THE PRACTICE?

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from our reception.

On the form, you will be able to say which practitioner you would prefer to see.

Please note that without all of these forms completed we will be unable to process your registration.  You are also required to show photographic ID or proof of living in Darwen at reception on returning the forms to us.  Please also ask for a new patient pack which has some useful information you may need.
