If you would like to register for PATIENT ACCESS so you can view some parts of your medical record, book routine appointments or order repeat prescriptions all online - please download a registration form to complete or alternatively ask at our reception for a pack.

Repeat Prescriptions
If you are on longer term or repeat medication and your regular prescription is due, please tick the items you require on the tear off slip on the right-hand side of your previous prescription and put it into the box provided on the reception desk. Please do not re-order items that you do not need.
Alternatively you can may post it to us, fax it to us on (01254) 226689 or click on the link on the above for the repeat prescription form.
If you have mislaid your reordering slip, a new one can easily be left for you to pick up by reception. Our reception staff will be pleased to help you with any enquiries you may have about obtaining prescriptions.
We encourage our patients to request repeat prescriptions in the above way rather than over the telephone wherever possible to help keep the phone lines clear for patients trying to get through with more urgent problems.

Prescription Fees
Please contact your preferred pharmacy to sign up to EPS.
From 1 April 2019, the charges are:
· Prescription (per item): £9.65
· 12-month Prescription Prepayment Certificate (PPC): £111.60
· 3-month PPC: £31.25
If you will have to pay for four or more prescription items in three months or more than 14 items in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is £9.65, whereas a three month PPC will cost you £31.25 and a 12 month PPC £111.60.
PPCs are available by 10 monthly direct debit instalment payments. The certificates allow anyone to obtain all the prescriptions they need for £2 a week.
